A $45 deposit is required to reserve the initial consultation. The deposit is non-refundable if the appointment is missed without notice. Please do not submit sensitive medical information through the general contact form. Health details belong on the client intake form, which should be printed and returned using Harbor & Haven’s instructions, not through this form or ordinary email.
